It's not a name, it's a title. A name, you're given at birth. I doubt many parents would just slap the name "Doctor" on their kid.

Konzen gave me my name long after I was born, 'cuz I asked for one. Maybe mister Doctor never got one and made up his own. Ya don't know that.

Well, I guess anything is possible. I just know on my world, it'd be hard to be part of society, with no name.
